NOTICE
Per Texas House Bill 4224 (89th Regular Session), which went into effect on September 1, 2025, and Texas Health and Safety Code Chapter 181, the following information is provided as guidance for how a person may request their health care records from MHMR Concho Valley (MHMRCV).
(1) CONTACT MHMRCV
Patients/Consumers currently in services with MHMRCV:
• Call MHMRCV by telephone @ (325) 658-7750 or (833) 406-0857 and asked to be transferred to your assigned Case Manager/Service Coordinator; or
• Arrive in person at the MH or IDD clinic where you receive services and ask to speak to your assigned Case Manager/Service Coordinator.
Former Patients/Consumers (not currently in services with MHMRCV)
• Call MHMRCV @ (325) 658-7750 or (833) 406-0857 and asked to be transferred to the Mental Health (MH) Records Department or IDD Records Department.
• Arrive in person at the MH or IDD clinic where you receive(d) services and ask to speak to someone in the MH or IDD Records Department.
(2) REQUEST YOUR RECORDS
• Ask for copies of your records.
(3) RELEASE OF INFORMATION
• Complete an MHMRCV Release of Information form.
(4) PROCESSING TIME
• Processing requests for records can take up to 10-15 business days to complete.
(5) PICK UP OR DELIVERY OF RECORDS
• Pick up a paper copy or electronic copy of records at MHMRCV or request that the records be mailed to you.
(6) FEES FOR RECORDS
• Per MHMRCV Procedure #4.10.00.01, the Center reserves the right to charge for the reproduction of records whether requested by the patient/consumer, their family, or an outside agency/provider, as allowed by the Texas State Board of Medical Examiners.
• A reasonable fee shall be:
> No more than $25.00 for the first 20 pages;
> 15¢ per page for every copy thereafter;
> In addition, a reasonable fee may include actual costs for mailing, shipping, or delivery;
> If an affidavit is requested, certifying that the information is a true and correct copy of the records, a reasonable fee of up to $15 may be charged for executing the affidavit.
